La basílica fue fundada por el emperador romano Constantino I sobre el lugar de enterramiento de San Pablo, donde se dijo que, después de la ejecución del apóstol, sus seguidores erigieron un monumento, llamado cella memoriae. Esta primera basílica fue consagrada por el Papa Sylvester en 324.
La Vía Apia era una calzada romana utilizada como ruta principal para suministros militares desde su construcción para ese fin en el 312 a. C. La Vía Apia fue el primer camino largo construido específicamente para transportar tropas fuera de la región más pequeña de la gran Roma (esto fue esencial para los romanos).
